Take the State of Utah Community Needs Assessment. The Utah Department of Health and Human Services is dedicated to ensuring all Utahns have fair and equitable opportunities to live safe and healthy lives. This survey is open to general Utah community members over the age of 18, specifically people over 60, adults with disabilities of any kind, and caregivers or people that support an older person or adult with a disability. We are also interested in hearing from people that speak a language other than English and can provide translation services if needed.

The purpose of this survey is to learn about what services are available or not available in your community to assist you with living a safe and healthy life. By completing this survey, you will help us understand your needs and knowledge of the services and resources available in your community.

Your participation in this survey is voluntary, meaning you can skip any questions that you are not comfortable answering. The answers you give will be kept private and confidential. All information will remain anonymous and secure. Survey results are combined and only reported overall or in large groups. Your name will never be connected to your responses. The results of this survey will be delivered to the Division of Aging and Adult Services, so they can better meet the needs of people across the state.
