Homeless Prevention Application
Procedure
Provided services include;
- Rent, Security Deposits, and Utilities
- How to plan and keep budgets
- Help resolve problems with landlords
Provided payments are no more than;
- One month's rent - $450 per family
- Paying utilities - $300 per family
Qualifications
Families must meet all requirements below;
- Must have lost of home or about to lose their home or their utilities due to past-due payments that result from an event (crisis) that happened beyond the control of the family.
- Must be able to keep their housing or utilities or find new housing with a single rent, deposit, or utility payment.
- Must show how it will make past due payments and pay future months' rent or utility payments after the crisis has been solved.
- Must have already tried all other means of getting the money to pay or tried to set up a repayment plan.
- Value of the household assets and things the family owns cannot be more than $2,000. Including assets and possessions that are immediately available to the family members.
- Payment is available once in a 12 month period.
- Must have at least one dependent child (under 18) living in their home
- Household total income per month cannot be more than 85% Standard Needs Budget, based on the household's size.

Please print application or stop by to one in person. You can email the application to Rosa Vargas or drop it off in the Administration Office located at 117 S Main Street in Monticello, UT. You may also apply below. For farther assistant or question please call our office.
