Alternatives Program
The Home and Community Based Alternatives program provides a comprehensive array of quality client centered services. The services are delivered in a variety of community settings designed to provide a choice of service delivery options to the eligible client who can continue to live in their own home, if their needs for social and medical services can be met. Home and Community based Alternatives services contribute to improving the quality of life and help to preserve the independence and dignity of the recipient.
The objective of the Older Americans Act Title IIIB Services it to provide services to frail older clients, including the older client who is a victim of Alzheimer disease and related disorders with neurological and organic brain dysfunction, and to their family.
Eligibility
To be eligible to participate in the program a person must:
- Be 18 years or older
- Be at risk of nursing home placement
- Have health and personal needs which can be adequately met in the community within the established cost limits
- Have low income and minimal assets
Cost financial eligibility for the Alternatives program is determined by the Area Agency on Aging Case Manager. Individuals who qualify for the program may be required to pay a small fee based on a sliding fee schedule.
Donations are always appreciated!
This program offers in-home services to persons to enable these individuals to remain in their own home for as long as possible. If these services were not available, older adults who have health, mobility or functional limitations would not be able to continue living in their current living arrangements.

- Residential and Nursing Facility over-night stays
- Home Health Aides
- Homemaking
- Home Modification
- Rented and purchased equipment
- Senior Companion
- Personal Emergency Response Systems
